@RR There are several stakeholders who caused this.
#1 being the structural engineering company who did the design. Probably zero investigation into the structure (to save money).
#2 being the developer putting pressure on the master engineering firm (JMV Associates) and the Prime contractor to meet a deadline.
#3 Prime contractor's "means and methods" to do the work. A lot of times they cut corners and use this as an excuse, bypassing engineering standards.
#4 "Independent inspection or 3rd party inspectors". Usually pretty worthless. They show up to the site for a short while and bitch about tiny ticky tacky shit while ignoring those YUGE CRACKS IN THE COLUMNS. Then go do a 3-hour lunch.
#5 Master engineering firm (Engineer of Record (EOR). Takes all the credit (and money) because they are a minority owned business that doesn't do any of the work (farms it out). Blames everyone else if the project fails.
